What is a Pacemaker?

A pacemaker is a small, battery-operated device implanted under the chest skin, near the collarbone. It consists of:

• A pulse generator – a small unit that houses the battery and electrical circuits.
• Leads (electrodes) – thin wires placed inside the heart to deliver electrical impulses.

The pacemaker monitors the heart’s electrical activity and sends electrical signals when needed to maintain a normal heart rhythm.

It is used to treat conditions such as:

• Bradycardia (slow heart rate) – when the heart beats too slowly to pump enough blood.
• Sick sinus syndrome – when the heart's natural pacemaker (sinus node) does not function properly.
• Heart block – when electrical signals between the heart’s chambers are delayed or blocked.
• Atrial fibrillation (AFib) with slow heart rate – to prevent excessively slow heartbeats caused by medication.
• Heart failure (in some cases) – to help improve the heart’s pumping ability.

Symptoms and Causes of Irregular Heartbeats

Common Causes of Slow or Irregular Heartbeats:

• Aging – the heart's electrical system deteriorates over time.

• Inflammation or infections – myocarditis or other conditions affecting heart function.
• Certain medications – beta-blockers and heart drugs can slow heart rate.

How is Pacemaker Implantation Performed?

1. Pre-Procedure Preparation:

• Patients undergo ECG, echocardiogram, and Holter monitoring to assess heart rhythm.
• Some medications may need to be adjusted before the procedure.
• Local anesthesia is administered to numb the area where the pacemaker will be implanted.

2. Pacemaker Implantation Procedure:

• A small incision is made near the collarbone.
• Leads (thin wires) are guided through a vein into the heart, positioned using X-ray imaging.
• The pacemaker is connected to the leads and implanted under the skin.
• After testing the device’s function, the incision is closed with stitches and a bandage is applied.

3. Post-Procedure Recovery:

• Patients stay in the hospital for a few hours or overnight for monitoring.
• The pacemaker is programmed to suit the patient’s heart rhythm needs.
• Detailed instructions are given for wound care and activity restrictions.

Types of Pacemakers

1. Single-chamber pacemaker – Sends impulses to either the right atrium or right ventricle.
2. Dual-chamber pacemaker – Regulates signals between both the atria and ventricles.
3. Biventricular pacemaker (CRT) – Used in heart failure patients to improve the heart’s pumping function.
4. Implantable Cardioverter Defibrillator (ICD) – A combination device that also prevents life-threatening arrhythmias.

Recovery & Post-Implantation Care

• Rest for a few days to allow the incision to heal.
• Avoid lifting the arm on the implant side for 4-6 weeks.
• Regular follow-ups to check pacemaker function and battery life.
• Avoid strong magnets and electrical devices that may interfere with the pacemaker.
• Inform healthcare providers about the pacemaker before MRI scans or surgeries.
